STANLEY FELLOWSHIP APPLICATION

PLEASE BE SURE TO FOLLOW THE INSTRUCTIONS OUTLINED BELOW WHEN

COMPLETING THE APPLICATION

The attached application is to be completed in order to apply

for a Stanley Postdoctoral Research Fellow position.  This application is

the official fellowship application required by the Johns Hopkins University

School of Medicine.  If you have a different application, please discard it

and use the application that is attached to this page. 

Each section must be complete and legible or your application

will be deemed incomplete and returned to you.

Do not refer to an enclosed curriculum vitae in lieu of

completing a section.  A CV does not usually contain all the information

needed (e.g., complete dates, addresses, names, etc.)

If a section does not apply to you, write “N/A” in the

section.  DO NOT LEAVE ANY BLOCK BLANK.

In Block 25, Medical references are requested.  Please

write in “N/A”.  However, we do request that you have three (3)

letters of reference sent to us in support of your application.  These

letters must be on official letterhead.  They can be faxed to us at

410-9555-3723, but we will not accept any letter by E-mail.

On the Continuation page, please put in a “personal”

statement regarding your application for a Stanley Fellowship position. 

We will need degree verification from your university/college. This

must be sent directly from the university/college.  Please contact us

if there is a problem with this requirement.  If you have fulfilled all

your degree requirements, but have not as yet received the official degree, we

will accept a letter form the university/college indicating this fact.
